What the metformin aging trial actually tested
Metformin is a widely used, inexpensive drug that lowers blood sugar in people with type 2 diabetes. Because it also affects cellular energy pathways, researchers have long wondered whether it could slow aspects of biological aging — the gradual decline in function that raises the risk of many chronic diseases.
The Antecedent Metabolic Health and Metformin Aging Study (AMHM) asked a more specific and unusual question: does metformin help everyone equally, or do its effects depend on where you start? The investigators hypothesized that metformin might *improve* insulin sensitivity in people who are insulin resistant, while potentially *reducing* it in people who are already insulin sensitive. "Insulin sensitivity" refers to how efficiently your cells respond to insulin to take up glucose; poor sensitivity ("insulin resistance") is an early step toward metabolic disease.
How the study was designed
This was an interventional Phase 3 trial run at two sites — the Oklahoma Medical Research Foundation and the University of Wisconsin–Madison — enrolling 166 participants aged 40 to 75, of any sex, who did not have type 2 diabetes. Healthy volunteers were accepted.
Participants were first classified as insulin sensitive or insulin resistant. Within each group, half received metformin and half received a placebo tablet over a 12-week course, with neither participants nor researchers knowing who got which (double-blind). Before and after, the team measured insulin sensitivity, glucose regulation, mitochondrial function (mitochondria are the energy-producing structures in cells), and biomarkers of aging. The trial ran from July 2020 to March 2025.
Is metformin approved as an anti-aging drug?
